I have had similar problems. The fix that worked for me was append the JkMount /*.do ajp13 at the end of the httpd.conf file for Apache. This seemed to solve the problem I was having. Hope this helps.

I didn't have the problem running in standalone using the localhost:8080 only with mod_jk.
