It's not an "app" but POIPond is a great tool for making/editing POIs
quickly and easily.
It's a mobile application that runs on pretty much any phone (Android,
IOS, Windows Mobile, etc.)

+1 to Pushpin. It is very useful app for iPhone.
But as Robert says, it is not open source.
So as alternative, POI+ for iPhone is my concern.
