Do you have backups that would show the proper contents of /etc/pam.d? It
looks like ssh is tripping up on PAM hooks.

Not TCP wrappers.
Webmin uses a separate one login.
I have the same experience from many clients.
I deleted the keys on the host and recreated new ones. I then updated my
client's key accordingly and I have the same experience.

PAM = Pluggable Authentication Module and usually involves setting up the
server to authenticate against other services and directories instead of
locally. I can't imagine that's you're problem unless this server is
authenticating with another server or service.

TCP Wrappers is usually in /etc/hosts.allow and /etc/hosts.deny if I remember
correctly. If you don't see any rules listed there, it's not TCP wrappers.
To set up a root password (when you have console access) you just:
sudo passwd root
and create a new password for the root user.

server and they are identical.
I am not sure what/how to investigate TCP wrappers.
