You can easily reproduce on a yakkety livecd, you just need to delete
the ubuntu entry on the facebook account to get that page which asks for
confirmation of the rights you want to give to "ubuntu"

It was after I entered the username and password. If you look closely
at the picture you can see where it shows my login but won't let me
click through since the window is too small.
