I guess the code you posted is used inside a LOAD_REGISTER, right? But
where do you get your data from? A CONTENT object?
In this case you could do something like
10 = LOAD_REGISTER
10.counter = 100

specify time of a publish date. I know TYPO3 4.6 allows that natively
but what about 4.5.x? Is there an extension able to do that (cannot find
one) ? Other suggestions ?

We recently upgraded our typo3 installations to 4.5.8.
When I go to the extension manager and pulldown Import Extensions
the page loads up fine.
Then when I try to search in Look up extensions box and hit the
Look Up button I get an instant error message.

Problem is that there was no information available at the point of
creating the cache entry, how long the content is
