Same problem here, after a fresh install of 12.04, one reboot and
leaving the system running for several hours. kill -9 helped.

Actually, with Gutsy Gibbon (fully updated) I still have the problem:
1) Fresh install
2) Right-click on Clock docklet, choose Adjust Date/Time
3) Choose Keep synchronized with internet servers in the Configuration list
4) Confirm to install NTP support
Now the dialog gets rid of the date widget
