The side buttons (BTN_EXTRA/SIDE or buttons 8/9 in Xorg) are mapped to
forward/back (Alt+Right/Left). So it sounds like the wheel is getting
misinterpreted as that, sometimes.
I wonder if you have found either:
* A keyboard modifier stuck down, if not physically then in software.

But we are looking for similar errors that might be occurring during
locking. So next time the problem happens, please unlock the screen and
then run:

If lowering the resolution or refresh rate solves it then most likely
you need a higher quality HDMI cable.
I say HDMI cable because those usually have the most quality problems,
whereas DisplayPort does not.
