This can be fixed by turning off TCP sequence reordering on the Cisco
appliance. Please note this also affects your Mac, BSD and Windows
machines. You can turn off SACK on your host if you don't care about
performance.

First off, sorry if this comment is redundant.
I cannot say for sure whether the issue happened after an update. I run
apt-get upgrade very often and thus cannot peg it down to an update. I
have definitely not upgraded (dist-upgrade) my Ubuntu version.
I will give the latest kernel a shot, but, I
I don't think I can run apport-collect. So far, I've had it happen about
3-4 times over 30 hours of uptime. If it does happen again, _and_ I'm
able to start a vnc session, _and_ said vnc session works, I will
definitely update this bug report with the logs.

Mike, under the scenario you've described, there is no problem
installing GRUB.
It only fails if the call to grub-install() specifies a non-ext3fs
partition; and if /boot/ is ext3fs, that's a moot point.
Still a bug though..
