It's been almost a year since I've filed this bug. Is there anything I
could do to help solve it? I'm with 13.10 final beta over here, 2
versions ahead of what I was using when it first happened and it still
there.

I think a small improvement would be at least show some feedback to

Yep. freeglut3 did the trick for me. Pobably adding it to list of required
packates will solve the case.
Fresh install of 13.04, used to play on 12.10.

I'm using quantal over here. There's a .0ubuntu19.1 quantal-proposed
package to download here, but nothing's changed, so I can't test yet.
I'm waiting for a commit to quantal, and as soon as it's available, I'll

Yes, disappears on both tabs. Is like I had disconnected it. I'm
attaching two screenshots with the phone connected and after switching
profile to "Off".
BTW, thanks for the pavucontrol tip! I'd never remember of this
workaround :)

set it to a half volume level on "sound-nua" applet.
However, if I disconnect/reconnect the headphone, the sound level is not
restored, the headphone is not set as default output on applet devices
