Update: I found that I can reliably reproduce the failure by running
stress --vm-bytes $(awk '/MemFree/{printf "%d\n", $2 * 0.9;}' <
/proc/meminfo)k --vm-keep -m 1
and then trying to hibernate. So I did a git bisect between
Ubuntu-4.2.0-34.39 and Ubuntu-4.2.0-35.40 and identified this commit[0]

I can confirm the part about the low speed. When copying a large file
(1.6 GB) via Nautilus (2.22.2), the speed was around 4 MB/s, when I use
the scp or sftp commandline tools the speed is 11 MB/s. Nautilus never
froze during a copy operation though.
