I am updating this as invalid, because on further check it came to light
that the ISO was improperly downloaded. an MD5Sum check + ZSync ensured
that this was rectified...

This is a decade old laptop and just before install saw a memtest error
... probably something in RAM. Good chances that the crash is because of
that. This bug is posted in the rare chance if its helpful to change
things such that minor RAM related errors can be worked
I can confirm that the bug exists (and has a simple workaround).
Mine is an updated UbuntuMate (15.04) and while trying to create a new Bootable
USB pen drive got the same error. As mentioned elsewhere, opened up the pen
drive in fdisk, removed all partitions, created one partition as 'c' type
I have the same issue (Save button is grey) and I have no certificates
to install. My VPN requires me to type username/password to connect to a
Windows VPN setup.

I believe this thing happened, because phpmyadmin's dependency wasn't clearly
understood to Synaptic.
By mistake I installed PhpMyAdmin without MySQL-server and then while
uninstalling, checked the box for removing DB, which obviously failed.

I confirm that the steps provided by Ricardo (on 2011-05-13) are working
fine for Ubuntu 11.04. Just that you would need to install phpunit using
apt-get in the end, once again.

I also came across with the above error message while using ssh.
I tried ssh in 2 machines in a loop. Only 1 showed(machine 21, below) the
error. And in that machine(21)i had changed the password once before i ssh. So
i think the error may be because of the password change.

Since then, there have been a few updates (security / automatic updates)
and although all install successfully, but all fail at the post-install

The gnome-system-monitor application window is gibberrish.
