I had the issue pop up again today. Kernel version 4.40-45-generic. I
have 3 servers, only one of which is having issues. All of these servers
are running the same BIOS version. These servers are as close as

It appears to be intermittent. We have had the same chassis and it works
with no issues on 2 out of 3 of the same chassis. But the third would
experience issues with seeing both drives and setting them into soft
RAID 1.

Hopefully this is enough information. Once the NVMe drives are installed
the server comes to a halt and won't finish the apport-collect. Then
after a reboot it no longer has the NVMe error.
