I think I have this same bug. I originally suspected that it had something to do with chrome, since it always seemed to occur when I was browsing the net, but after reading your report, I now think the more likely candidate is eclipse, since I've always had eclipse open when the bug occurs.

I can also confirm and reproduce this bug with kernel 2.6.31-14-generic and openafs 1.4.11+dfsg-1. I tried using openafs compiled for kernel version 2.6.31-14-generic by installing openafs- modules-2.6.31-14-generic, but I encountered the same bug. -- evince makes openafs to kernel oops
