The card is a pluggable PCI card. The only other USB device attached is
a HP Printer. I have tried to remove all USB devices though, and the
problem persists. I don't have a wired network to check to see if this
problem affects wired connections as well.

The motherboard is the ECS AMG690GM-M2. It has integrated video (ATI
Radeon, I believe). Here's the output from lspci-vvv
As for the other settings, I'm using Ubuntu 9.04, and it's a pretty vanilla
setup. I am using compiz, I believe, because I am getting the custom display
effects.

After waking up from Hibernation, the wireless network is completely
inaccessible. When you click on the network icon in the menu bar, you see
Wireless Networks / Disconnected and no way to reselect the previously used
