I have the same problem. It would be nice if there was a "Don't use
theme" option for firefox.

After looking into other problems I'm having, I found that this problem
is related to fglrx. Apparently, the fglrx driver doesn't load properly.

I had the same problem using the following icon packages:
human
crux
dropline nuovo
dropline neu
I'm sure there others. The similarity is the location of the extra pics
around the icon (the desktop, lock, etc.)
