It seems that it is not a single problem that happens on analog surround

I tried using Bluetooth audio, then set custom balance. The same thing
resets when the volume gradually turns down and reached 0 volume. Didn't
reset when I manually mute it with a switch or through a button.

The problem is that the bgrt theme requires resources from
/usr/share/plymouth/themes/spinner, but /usr/share/initramfs-
tools/hooks/plymouth only copies in the default theme directory (bgrt)
plus text as a fallback. Hacking the initramfs-tools hook to always
include "spinner" in currthemes avoids
The prompt is visible if you hit Esc, and (at least sometimes?) shows up
on the text theme when you hit Esc again. After entering the password,
hitting Esc a couple of times gets you into the spinner theme.

As per instructions on bug reporting, I ran lsb_release -rd, which printed
18.04.3 LTS on the first line,
Release 18.04 on the second line.
I'm sorry, I'm out of my depth when it comes to kernels. It is purely a
test environment and I'm happy to give remote access and/or follow
instructions, but, I'm really not sure how to do this.

Host Server - Dual Xeon E5-2650 v4, 768GB Memory, ESX 6.5.0
I created a VM with 1vCPU, 2GB Memory, 16GB Hard drive.
I installed Ubuntu Server 16.04.2 (latest ISO I had on hand), installed
fine.
I then ran apt-get update, apt-get upgrade... upgrade failed as repos
Power cycling does not appear to trigger the bug, as well as attempting
to recreate the original circumstances (turning off wireless through
system tray + shutdown).
