@Adam, yeah, I noticed a while ago and reported it to operators at
Canonical, but it seems to be down still.
http://voices.canonical.com/user/128/ seems to be up though.

I don't know the ins and outs of HDMI hotplugging with pulseaudio, but I can
suggest a workaround -
Create a file named /etc/modprobe.d/blacklist-hdmi.conf with the following line:
blacklist snd_hda_codec_hdmi
Then, run:
sudo depmod -a
sudo modprobe -r snd_hda_codec_hdmi
