This is also affecting me. I had been using Ubuntu 12.04 with a slightly
older version of the nvidia driver downloaded from nvidias site. This
past weekend, I installed a new SSD - and new Nvidia drivers 304.51 -
and I'm just experienced it for the first time, and it's happened twice

I have the same problem: Fresh install of 12.04 x64, Nvidia 304.37
driver.
I didn't have this problem in 10.04; actually I would say it's started
in the last few months but I can't put an exact date on it.

I should point out that I was NOT affected by the related bug on 10.04 -
I had it running for years without issues, 64bit with nvidia driver. The
driver version was 195.36.24-0ubuntu1~10.04.2 for 10.04 and for 12.04 it
is 295.40-0ubuntu1

that error message has nothing to do with the crash, it just shows that
the xserver has died and pulls the rug beneath gnome-session.
