This bug is fixed[1] in upstream. The patches[2] are currently available
in linux kernel usb-next[3] git branch and it will be included in next
kernel major release which will be v3.7.

Further testing results. With mainline kernel v3.6-rc1 device is active
after every reboot but after suspend/hibernate device is missing. This
is the log containg output before and after pm-suspend command.

I've installed and tested mainline kernel v3.6-rc1 and the issue remains
unchanged. I also tested mainline kernel v3.5.2.
