This bug is still present on ubuntu 14.04 LTS, I can't guess how to
trigger it, but it does frequently.

2. Select play on the RB insert
3. hover the mouse around the play button note the thin square
4. pause and do the same.

I see the same square outline around the play/pause symbols, in totem's
on-screen-display. Like when watching a video in fullscreen, and
hitting the spacebar for pause. I don't think that was there a couple
weeks ago, in Quantal.
Of course, if it's related, I'm not sure where to file this. :)
