Confirmed for Ubuntu 14.04, Compiz. Workaroud is to restart Firefox. One
way how to loose Firefox window decoration and block keyborad is to
invoke open new tab from another application. I.e. invoke testing page
from NetBeans. It happens randomly, not always.

I see the same exact behavior as timothee.
If I unminimize the window before clicking a link in another app, no
problems. It's the 'unminimize as a result of being called up by another
app' behavior that triggers the bug.
I also see this with thunderbird if it is minimized and I click a
"mailto"
I'm able to reproduce this problem with the following steps:
- start FF
- minimize FF
- open a link from another software (Purple or Thunderbird)
- maximize FF from the task bar, the decorator is missing.
Workaround:
- select FF with the missing decorator, ALT+SPACE then Minimize, then ALT+TAB

Just to elaborate on my last comment, when FF loses it window decoration
and won't come to the foreground, it is the last active foreground
window that continues to receive the keyboard input. If that happens to
be something like a terminal or text editor, you can see it happening.

Reading through all of this I've had exactly this same bug since 11.04 I
think, or it may have been a later release. Anyhow I'm now running a
fully updated 12.10 x86-64 bit release, and the issue s still here. I
run in the Gnome fall-back session with Cairo Dock on a dual monitor

I've seen this bug lots recently, but always with maximized firefox
windows. However, just now I managed to reproduce it with a none-
maximized window. The window accepts both keyboard and mouse input but
has no decorations and cannot be moved with alt-dragging. It is almost
as if compiz has simply
