Currently when adding new keyboard shortcut:

* there is no hint when the shortcut should be provided, the window
simply says "Command Shortcut" (see screen) so it could be understood as
a summary/confirmation message

* if shortcut is not provided when window expects it, item is simply not
added at all, providing no useful feedback

This is quite frustrating for user not familiar with how the keyboard
manager works, it seems as if something was broken but it's not clear
what.
