Bug appears to exist upstream in version 3.11.0-031100rc6.201308181835.
Have added the above tags as requested. Basically rebooting into the new
kernel leads to the same issue with a complete freeze until the USB key
is unplugged and replugged. At that time, the system returns to normal.

It has happened on/off since precise, but as I was using the usb
adapter on my desktop I could just pull it out and put it back in very
quickly. Now it is on a server this bug is more significant!
