Yes, it still happens. I sanitized (semi-anonymized) the source
document, then created a data file with nine fake addresses, and merged
it, creating the same result. When I merge this document, I see two
copies of each image in the output file, with the duplicate image
floating near the top of

names and addresses. These names and addresses are fictitious and are
not intended to refer to any person, living or dead. ;-)
