the workaround is to wait before entering the password that there is no
more activity in the USB drive, in may case wait until the light in the
USB drive is not blinking any more.

G - when everything does work, the "installing bootloader" phase takes so damn
long (without any indication whatsoever of the percentage of completion) that
you may think it's stuck.
And so does the "creating etx2 filesystem in persistence file" phase.
This tool should be replaced with something
Definitely the case as described in comment 2.
I was getting (B) systematically now, and after reboot it did prompt me for the
password as expected.
I forgot one more possible outcome, however:
E - CRASHES when installing the boot loader (after taking almost as long
as copying 3GB worth of files
F - when erasing disk, you get an error, something like there was no
reply from somebody

I strongly suspect what actually happens in case (B) is that it fails to prompt
for the administrator password, so it waits forever as if it was waiting for
you to insert the password to gain root privileges.
This is just a guess, though (based on the fact that when everything works
fine, you us
