I think Christopher is onto something... I have Unity on both monitors -
the left (Primary) monitor opens the applications multiple times,
without issues. However, the right hand (secondary) display only opens
an application once, as detailed above.

Some additional explanations:
I have visible launcher on left side of may right monitor.
But launcher is NOT visible on may left secondary monitor, beacuse I turn off
showing launcher there.
I have nvidia 331.38 driver installed. Driver represents my both
monitors as one X screen

I can confirm this bug on my 2012 Mac Pro (Nvidia binary driver 331.38 from
nvidia-331-updates (proprietary)).
The same bug doesn't happen on a Asus N56VM (Nvidia binary driver 331.38 from
nvidia-331 (proprietary, tested)).
