Public bug reported:

Unity loses windows of running applications when multiple monitors are
in use. To reproduce:

- set up dual monitors, 4 virtual desktops
- open app
- right click on title bar and select "move to workspace X" (this menu also 
claims the window is on a different workspace than it really is)
- window disappears and launcher indicator goes away indicating that the 
application closed
- the actual application is still running (can be verified with ps)
- the app window has become inaccessible
- clicking on the launcher icon launches a new instance of the app, the old one 
keeps on running in the background limbo
