With regards to the original problem statement, apt-get clean fails
because there is nothing in /var/cache/apt. However, apt-get update
works fine.

Also removing everything in /var/cache seems a bit extreme, a more
concise test case would be removing things in /var/cache/apt/.

emptied apt doesn't recreate in all cases the missing directories for
example with the command "apt-get clean". Here is an example output:
