It is still nearly unusable without this patch (my macbook gets up to
90°C - even with the proprietary driver it is at 74°C

I don't have a Macbook so I can hardly review this patch; what is for
sure is it would truly benefit from being brought up to the upstream
mailing list again; so that it can have sufficient review before
including in the Ubuntu and Debian packages.

Using the nvidia driver drops the battery life of the macbook pro down
to 2 hours so yes usability is compromized without this patch.
