I booted last nights Ubuntu-GNOME 15.04 cdimage, and the titlebar
problem is not present there. I haven't used it enough to see if it also
fixes the other graphical regressions.

I tried the updated mesa from xorg-edgers. Unfortunately that doesn't
solve the issue and makes the other issues much worse (now almost every
app is missing some glyphs and icons, and windows have a weird glow
around them where the shadow would otherwise be).

That might be true, as I just downgraded to xf86-video-intel version
2.99.910-0ubuntu1.3 (i.e. the version from 14.04) and I still get the
titlebar problem. I'll look for some time to experiment with different
Mesa versions.

Managed to capture a screenshot with sleep 10 import -window root.
Here you can see where the maximized window is missing the right hand side of
the title bar, and also some weird stuff on the left.

I updaated to xserver-xorg-video-intel
2:2.99.916+git20141215.99537089-0ubuntu0sarvatt~utopic and problem #1 is
still immediately visible. I'll update in a few days time to let you
know if 2 and 3 are still around.
