Public bug reported:

First of all, apologies if this is filed against the wrong package, but
this seemed like the most pertinent one.

When you upgrade from utopic to vivid on an OS X machine running
Yosemite, the upgrade procedure overwrites the boot code somehow. When
booting the computer will print "booting in insecure mode" or something
like that and boots immediately to Ubuntu. The only way to boot to OSX
is to hold alt when booting and then selecting "efi boot" from the list
of items. This brings up the proper rEFInd boot menu.

The expected behaviour is that after the update booting happens via
rEFInd just like before the upgrade.
