I have the same problem. On 1st load menu bar is there. For me to use
the menu bar I make the application full screen then the menu appears
when I work from smaller window the menu disappears again.

I can't reproduce any of these problems using Kubuntu 14.04. I suspect
your issue is related to something about how Unity and PyQt4 interact.
Can you try another PyQt4 based application, perhaps calibre, and see if
you have similar issues.
