Public bug reported:

Running from on-disk ISO file. Got an error that it could not configure
apt-get to install updates from CD (reasonable as there was no CD, just
a loopback ISO). Installer is not creating the initrd.img file, just a
bad link in the root directory. Not sure if that's related.
