I was trying to install Ubuntu 14.04.3 on kalelp3, and using multipath disk 
mpath0 as the installation device. The installation was successful. But after 
the installation when the machine reboots itself, it shows:

 No OS image was detected by firmware.
 At least one disk in the bootlist was not found yet.
 Firmware is now retrying the entries in the bootlist.
 Press ctrl-C to stop retrying.

I entered SMS and noticed that mpath0 is in the device list as item 7
and 9, but it is not set as the 1st boot device. Actually, no device is
set as the 1st boot device after the installation, so FW cannot find a
bootable device.

Manually booting from item 7 shows that this is the multipath disk we
installed Ubuntu on. I think this device (or both 7 and 9), should be
set as the 1st boot device.

We have another lpar hitting this problem, too. I have collected and uploaded 
the log file, please take a look.

Here is the steps for install the lpar:
1.) Boot from that network port & You should see this grub menu when it loads

GNU GRUB version 2.02~beta2-9ubuntu1.2
Install Ubuntu 14.04.3
Install Ubuntu 14.04.3 with Multipath    <-----select this one 
Install Ubuntu 14.04.3 w/ preseed and Multipath

2.) Language : English
3.) Country : United States
4.) Configure the network
hostname: miglp2

5.) Ubuntu archive mirror country: choose United Kingdom
enter information manually
United Kingdom

6.) Ubuntu archive mirror:
ports.ubuntu.com

7.) http: proxy information:
http://10.33.4.4:3128

8.)*Set up users and passwords
Full name for the new User: lte
username for your account: lte
Choose a password for the new user: {use what we do for the root user 
don2rry......Do this twice}

Answer yes to use weak password
Encrypt home directory; No

9.) Configure the clock
answer yes to use Chicago time zone

10.) Partition the disks:
Guided - use entire disk
Here I usually mpathx that has biggest size
Answer yes to format the disk(s)

11.) Installing the base system/Configuring apt/Select and install software
How do you want to manage the upgrades on this system: No automatic updates
installing Grub Loader

12.) Finish the install
Answer yes to set the system to use UTC.

13.) Finish the installation

This problem was first  seen on last Friday, and it is a regression bug.
