In my case it helps to restart the service lightdm (sudo service lightdm
restart) - this works around 50% of the time, but in some cases the
system freezes afterwards and a hard reset has to be performed.
Another issue is that if the second monitor goes to sleep it is not
waking up afterwards.

Same problem in 16.04 with "Intel(R) Core(TM) i3-3110M CPU @ 2.40GHz"
Lenovo Laptop. While HDMI cable is connected, TV shows "no signal".
Comfirming that:
- LG TV's HDMI ports work.
- HDMI cable works (under Win7).
- Laptop can detect active HDMI connection (through xrandr)
Terminal output:
~$ xr
Same problem with a Notebook and intel graphics and the "nomodeset and
idle=nomwait" option not working...

HDMI connection not working on
I have had some kind of success on this front. In order to get Lubuntu
to boot and install I added nomodeset and idle=nomwait in the linux
parameters in grub. I found here
http://ubuntuforums.org/showthread.php?t=2042773 that nomodeset is the
thing that is preventing the HDMI from working. I tried
Status changed to 'Confirmed' because the bug affects multiple users.
