This bug still exists in Ubuntu 9.04. On my DELL 9400 the connection is
lost during heavy load (like downloading updates) and it's impossible to
reconnect without doing a reboot.

This could be the same as bug 159064. I usually don't get any
disconnects during use, only have problem with the initial connect.

since the LED works with this module.

I'm having the same issue with ipw3945 on a Dell XPS m1330 -- initial
connection is good. Sometimes will recover a lost connection "hiccup."
However, after a while (1-2 hours later) the next hiccup will produce
the failure to reconnect. The WiFi LED light stays on and becomes
unresponsive to the
"Disabling/re-enabling networking isn't an issue."
That was unclear. What I meant is that doing so doesn't appear to do
anything, except freeze the nm-applet.

Nope stay away from 1.2.23. Module loads fine but when it initiates an
scan everything freezes. Reverted to 1.2.22 again.

Got these issues as well on a Toshiba M400, upgraded to iwl3945 1.2.22
manually a few weeks ago. Although this version does recover after a
minute or so there are still intermittent freezes to cope with. Gonna
try 1.2.23 this evening and see if things got sorted out.
