I wasn't able to recreate this on a bionic-queens deploy. My deployment
was a bit different, quite a bit simpler. Results are attached. I did
notice that you're using bluestore so it might be worth testing with
that. Again logs or a small recreate scenarios will be useful to help us
figure this out

libvirt silently fails to attac
Interesting that you have no cinder-ceph volume at all from libvirt's
POV.
We no more have to look at/inside the guest (e.g. lsblk).
The breaking point seems to be between openstack and libvirt.
Openstack thinks it has told libvirt to attach the volume

I have no ceph system around, but I asked for how the storage appears there.
Once I get that back we'd have something to compare.
Further that log almost seems as if the guest would die after trying to attach.
But seeing that you report `lsblk` from the guest it must be alive right?
