Might have been some GUI update issue, that's hard to track that down
though. You could install kdebase-dbg-kde4 and kdebase-workspace-dbg, so
maybe something shows up in ~/.xsession-errors if it appears again.

This works for me in Konqueror now, but I can swear that I did remove the
button (just like in your screenshot Harald), but the button did not disappear
from the actual toolbar.
I'll comment here if it happens again.

Yes, removing it from the toolbar-configuration-window works for me too.
