Public bug reported:

I have installed from this CD before, and everything went smoothly. The
prior install was against an internal SSD.

I am now installing on a USB stick, and had to remove /usr/lib/ubuquity
/apt-setup/generators/40cdrom to fix a previous failure. (I actually
moved the file under the home directory. I hope that didn't cause the
problem.)

I will reimage the installation stick, reformat the destination stick,
and try again.

Title:
  Disk I/O error from Install. Removed 40cdrom from install disk to
  correct last reported error. I have installed successfully from the
  thumbdrive.
