Yes, the icon still remains :P I've noticed that when I remove an icon
from the KDE4 desktop by clicking its x button, the item is still
shown when I run ls ~/Desktop. Maybe they aren't going to release the
fix until KDE 4.0.4?

No, not related.
hangy's issue is caused by a unncessary translation of a configuration string
which makes plasma unable to read it's own configuration. A fix for this is
already in KDE and should soon become available in our packages as well.
The initial report however is only related to the

Probably related to this:
I got only one folder in ~/Desktop/ (nothing but that one folder in there,
actually) and since some update, an additional instance of the widget (I assume
those icons are widgets) is added on every start. So by now, I have got that
icon on my desktop like 15 times or
11 matches
