My problem was resolved after I deleted /etc/kde4rc.

I fixed this last week. It was a crack upload of kubuntu-default-
settings that caused this. Just remove /etc/kde4rc if you are still
experiencing this problem.

I didn't mention, I use kdm-kde4, but kdm-kde3 doesn't make any
difference, or running just X and then DISPLAY=0:0
/usr/lib/kde4/bin/startkde from a pure console login.
