As far as I know, shutdown and restart are not supported in KDE when not
using KDM. These options should be removed from Kickoff as well as the
dialog that pops up.

I go to the Leave menu and click on shutdown or loggoff or restart, next
appears the black box. In the black box only the loggoff option button
is visible, no matter the button you click in the Leave menu.

Can you describe what you are doing and which buttons don't work for
you? (Is it the buttons in the menu or the ones in the black box that
let's you choose between logout, shutdown and reebot?)
Does this still happen with KDE 4.0.3?
