It's true that this packages are fixing the volume problem, but I get
extreme choppy sound. Actually it's more like choppy silence ;).

Well, I found a solution for the problem. You just have to recompile the
alsa modules (here the problem seems to come from alsa). Let me add that
this kinf of problem doesn't exist on fedora 9. This what I did :
sudo apt-get install module-assistant
sudo m-a update
sudo m-a prepare
sudo m-a a-i al
Man, Canonical never tries to really help it's users.
This is a simple problem that Ubuntu has created because of it's early unstable
support--in a final release mind you, very unprofessional--of pulseaudio.
I'm about to ditch the distro for good.

I've tried your solution and in my case it only adds in gnome mixer.
Moreover, the headphones stopped work. When you remove "options snd-hda-
intel model=3stack" from /etc/modprobe.d/alsa-base, the full is still at
full volume at each startup but the headphones work.

Thanks Jeroen, it seems to work better (something changed in gnome
mixer. However the problem still exists : the canal "playback : ALSA PCM
on front : 0 (ALC861 analog) via DMA (pulseaudio)", that allows me to
control the volume, is at full volume at each startup.

Note that I am having the same problem (same audio hardware). I've found
a forum were the following solution was suggested:
to /etc/modprobe.d/alsa-base add the following line:
options snd-hda-intel model=3stack
This worked for me ...
