Solved - it was actually a bad speaker. Replaced: Fixed, currently
using all default sound settings from Ubuntu Install.

I do not have any audio from my left speaker, my volumes are turned up
completely. When I plug my headphones in, I have audio in both left and
right headphone.
I ran (without headphones)
$ speaker-test -Dsurround71 -c 6 -t
Same here, but only Front Left is working.
