I get this also, but only for a secondary USB sound device.
I have 2 volume-applets, the first works fine, the other spontaneously mutes
one channel, refuses to lock l/r, etc.
the mixer that functions is for a "HDA Intel"
the broken one is for a "Sound Blaster MP3+"

I also have this problem on a fully updatet Ubuntu Hardy. When I try to
adjust the volume, the volume just goes up and down seemingly randomly,
and one of the channels drops out temporarily while I keep adjusting the
volume.
I have a HP nx6325 laptop.

Here is a screenshot of 'volume control'. It is impossible for me to
raise the left channel as it refuses to move. Also if i click the link
button it sometimes makes it so i can raise the left channel a bit.

Bug is indeed still there on a fully patched hardy.

applets/+bug/126333 , which is just a display bug, this one actually
impacts sound output.
