Since the last update, I have this same problem. I don't use Compiz and
haven't done anything else to my computer.

I just had this problem, and it seems to be the Legacy Fullscreen
Support in the Workarounds of Compiz. disabling it fixed this problem,
and re-enabling it makes the problem come back 100% of the time.

This worked for me:
1. Disable Compiz
2. Close and open Firefox (the borders should appear again).
3. Enable Compiz
4. Close and open Firefox (the borders should be still there).
I rebooted the computer to see if It still worked and yes... they were
there.

I have the problem on after a upgrade from hardy to intrepid. I use compiz,
when i start firefox with
firefox --safe-mode and reset toolbars and controls, it works the next time.
