After looking through some forums I found a solution to this problem.
The problem was fixed by adding the following line to /etc/modprobe.d
/alsa-base:
options snd-hda-intel model=dell-m42

I am having the exact same problem as Neil Vilela. My headphones work
when they are plugged in upon start up, but no longer work once the
headphones have become disconnected. There is no "headphone" or
"surround" sliders for me as well. I am using a Gateway T-1628 running
Ubuntu 8.10.

The headphone jack works if I turn on the computer with it plugged, but
won't work anymore if I unplug and plug it again, no matter if I logout,
restart X or anything.

That also happens to me. I have a Gateway T-1625 laptop and there is no
headphone output since I've upgraded to Intrepid Ibex.
It does work when I turn on the laptop, but doesn't work anymore if I
unplug and plug the headphone again. There is no "headphone" or
"surround" slider in alsamixer here.
