Turns out "btusb" is replacing "hci_usb", so this is normal.
/etc/init.d/bluetooth was not set to start in runlevel 2, which was
causing this problem. Works great once this was fixed.

I forgot to mention I'm using a Thinkpad T61. I also tried to plug a
bluetooth usb dongle and it's not listed with "hcitool dev" either.
Both bluetooth devices work fine in Hardy.
