Ah. I found this thread

and looked in a little more detail at 'alsamixer -a 0'.
It turns out that while I had turned up the volume on the different
levels, they were muted for some reason. Once unmuted (by pressing m),
all is well now.

this was more user error that a bug.
it's confusing that all channels are muted, but I think the others have
reported a bug on that.
