Had the same issue with a fresh installation (not upgraded) of Ubuntu
12.04 (Precise). I could also fix it by changing the group of
/etc/shadow from root to shadow.

The problem resolved itself on two of my machines when upgrading to
Karmic release. One machine, however, still wouldn't unlock the screen.
I found the solution for my situation at bug #299717. I needed to fix
the group assigned to /etc/shadow.
The solution was to:
chgrp shadow /etc/shadow
I see the buggy behavior on three machines, all fresh installs between
Alpha 6 and Beta 1, and all updated on the same day to Beta 1.
I went through the bug at kde.org, and tested kcheckpass. I too fail:
rich...@laptop:/usr/lib/kde4/libexec$ ./kcheckpass

I just set up my screensaver lock and everything and was unable to
reproduce. Trying to trigger it and I am unable to right now.

I found an upstream report with this issue, however it was resolved and

I am seeing this behavior with Karmic Alpha 6 and Beta 1. My report is
in Bug #434276 (I swear I searched for a duplicate before logging a new
bug). The summary is that all permissions appear to be correct, but I
cannot unlock my screen.
After reading this bug report, I verified that xscreensaver

I had this issue at the beginning of the year and was totally lost, and
found out xscreensaver was kicking in, which I accidentally installed
one day during development stuff. Thanks!

Hi Harald! Tried adding the lines to xscreensaver with no effect, still locks
won't open.
In a way I wonder why do I have xcreensaver installed.. Can't remeber if I
installed some xscreensaver-savers to ksreensaver-package or something..
tips?

KDE4 screensaver lock doesn't accept password
Hey Risto!
- are the missing lines of /etc/pam.d/xscreensaver the problem?
possibly ;-)
Just add the lines and see what happens. I really don't think the KDE pam files
are at fault here, since I created them ... and I am quite confident of my work
;-)
This also would explain why your screen

I don't have screensaver enabled but when the computer is left alone for
long enough, the screen is locked then later also the screen is turned
off. Desktop computer so closing the lid etc doesn't apply here.
